Embarcadero Dentistry is committed to protecting the privacy of your protected health information (PHI).

We are required by law to maintain the privacy of your PHI, provide this Notice of our legal duties and privacy practices, and follow the terms of the Notice currently in effect.

How We May Use and Disclose Your Health Information

Treatment: We may use and share your information to provide, coordinate, or manage your dental care.

Payment: We may use and disclose information to bill and receive payment from insurance plans or other payers.

Healthcare Operations: We may use information to operate our practice, improve care, train staff, and conduct audits.

Special Protections for Certain Records (2026 Federal Requirement)

Certain records, including substance use disorder treatment records obtained from federally assisted programs, are protected under federal law (42 CFR Part 2) and may have additional restrictions on disclosure.

California Privacy Rights

California law may provide additional privacy protections, including rights under the Confidentiality of Medical Information Act (CMIA). Where state law is more protective than federal law, we will follow state law.

Your Rights

You have the right to inspect and obtain a copy of your records, request amendments, request confidential communications, request restrictions, receive an accounting of disclosures, and obtain a paper copy of this Notice.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with Embarcadero Dentistry at (415) 398-4400 or with the U.S. Department of Health & Human Services. You will not be retaliated against.

* Data Retention: We retain your information as long as you are subscribed to our
SMS service. You may request deletion at any time
* MESSAGE AND DATA RATES MAY APPLY. Your mobile carrier may charge fees for sending or receiving text messages, especially if you do not have an unlimited texting or data plan
* Messages are recurring, and message frequency varies.

* Opt-Out: You can opt out of the SMS list at anytime by texting, emailing, or replying STOP or UNSUBSCRIBE to admin@embarcaderodentistry.com or call (415) 398-4400.  After unsubscribing, you will receive a final SMS to confirm you have unsubscribed and we will remove your number from our list within 24 hours.
* You can send HELP for additional assistance, and you will receive a text including our Phone number, email and website. We are here to help you.
* Non-Sharing Clause: We do not share your data with third parties for marketing purposes. Embarcadero Dentistry will not sell, rent, or share the collected mobile numbers
